We’re here to help you through the claims process; quickly and efficiently. If you need to report a claim, please review your policy documents to identify the appropriate insurance carrier. Most claims are submitted directly to the carrier listed on the policy.

Locate Your Policy
Have your policy number, insured name & incident details handy.

Send Bass Your Loss Notice
Attach the completed ACORD form and email claims@bassuw.com

Receive Your Claim Details
After a TPA is assigned, we will provide you with the claim number and the adjuster’s contact information.
⚠️ Please note: We are unable to process the claim without a completed ACORD Loss Notice. To ensure your claim is handled as efficiently as possible, include as much detail as you can.
